How can I prepare for my first rheumatologist visit?

For over two years I've had increasing bouts of joint pain lasting longer and with more joints involved each time. I am finally seeing a rheumy for the first time next week. I'm feeling nervous with so much hanging on it. What can I prepare to help it be most useful? Thanks!
